Every five years, each program will conduct a more in depth self study and submit a Summary Report including:
- A compilation of the outcomes assessment progress reported in the Annual Outcomes Reports (Forms A & B).
- Analysis of the ten required assessment criteria listed below and described in more detail in the next section of this document. Five of the ten required assessment criteria are provided to programs each September in the Annual Profile. (Copy in the Appendix)
- Analysis of optional assessment criteria that the program faculty determines is useful additional feedback.

Step I: Review and Collect
At least one year before the Five Year Summary Report is due, each program should review the information in their PEP three ring binder and decide which of the required assessments for your program have already been carried out measure the outcomes identified by your program. The checklist on page XX of this document may be of use to you.
Step II: Identify and Carry Out Outstanding Required Assessments
The annual profile provides the assessment results for the first five required assessments. All professional and technical programs and academic departments are responsible for documenting that their curriculum is relevant and current. (See page XX) Professional and technical programs must have results from a market analysis to report. A consultant should be provided to conduct this market analysis.
Detailed information about each of the required assessments is found in the next section of this document.
Step III: Identify and Carry Out Optional Assessments
Review the list of optional assessments and decide whether any of them would provide information that would be of use to the program at this point. Detailed information about each of the optional assessments is found in the fourth section of this document.
Step IV: Complete the Five-Year Summary Report
Use the information collected in the programs PEP three-ring binder and the results of any required and optional assessments carried out by the program to complete the Five-Year Summary Report. Commentary on each assessment item is limited to three lines.
Complete an action plan, which includes any outstanding action, to be taken on the outcomes assessed over the last five years and any action warranted by the results of the required and optional assessments. Submit the Five-Year Summary Report to your division chair or supervisor. The division chair or supervisor is then to submit the report to the appropriate executive dean or vice president.
